All dogs in the care of What A Good Dog are required to have a Cardiopulmonary Resuscitation (CPR) or Do Not Resuscitate (DNR) directive. We want this information in order to proceed in a manner consistent with your wishes in the very unlikely event that something would happen to your dog when in the care of What A Good Dog, including any veterinary facility that would provide care to your dog.
CPR is the resuscitation of an animal that has stopped breathing or whose heart has stopped. Animals that survive cardiopulmonary arrest and have been successfully resuscitated (CPR) are critically ill and unstable. Management of the post-arrest patient requires vigilant monitoring and the technical expertise of critical care personnel. The care is costly and the outcome is uncertain.

I DO NOT want CPR performed on my dog. I understand that my dog will die in the event that he/she stops breathing and/or his/her heart stops beating. My wish is to have a DNR (Do Not Resuscitate) order for my dog.
I wish for emergency staff to perform CPR (Cardiopulmonary Resuscitation) on my dog if my dog suffers from cardiac or respiratory arrest. I understand that my dog may not respond to CPR and may die despite CPR. I also understand that if my dog responds to CPR it is possible that he/she will arrest again. I understand that the initial cost of CPR and the necessary aftercare will be substantial. I agree that I am responsible for all CPR and related hospitalization costs regardless of the cause of the arrest.

I am the owner or responsible agent for the dog named above. I have the authority to execute this consent. I am over 18 years of age. I have signed this agreement indicating my wishes for life support while my dog is in the care of What A Good Dog, including any veterinary facility that may provide care to my dog. I have had the opportunity to ask any questions I may have regarding this life support directive.
